Completed Work Payment & Service Fee Info

 As of 8/1/11

All equipment MUST be picked up within 15 days of completion.

After 15 days there will be a service fee of $10.00 for every 15 days until picked up or paid for.

All diagnostic fees are due at the time of dropping off equipment and before it is looked at.

All diagnostic and pick-up fees are due on or before time of picking up equipment.

Please print this form below to have with equipment at time of pick-up if you are having a diagnostic performed. This needs to be signed and left with payment if not already paid or previous arrangements have been made. If fee has been paid just simply sign and leave for pick-up person along with equipment. This is also the same for that will be filled out when dropping off equipment as well.
